Job Description:
Title RN II – Case Management Schedule Days, 10 hours Assignment
Details Contract: 13 weeks Guaranteed Hours: 40 per week
Requirements - Knowledge of InterQual criteria and Medicare
Conditions of Participation preferred. - Basic computer skills,
including Word, Excel, Outlook, and navigating the Internet. Roles
& Responsibilities - Assess appropriateness of admission status and
level of care using InterQual criteria. - Provide discharge
planning services, coordinating with patients, families, and
community agencies. - Collaborate with medical staff to assure
appropriate utilization of hospital resources and expedite
high-quality patient care. - Participate in occasional weekend
rotations as part of the schedule.
