GENERAL DUTIES: The Oncology Nurse Navigator acts as the primary point of contact after initial diagnosis and connects patients with the physician who will provide their initial treatment/care. The Nurse Navigator is responsible for providing education to patients and family members; monitoring and coordinating care with additional healthcare providers throughout the full spectrum, duration, of care, including diagnostic imaging, treatment and survivorship or end-of-life care; providing resources and support services information; and assisting with community outreach and education regarding health and cancer care.


Oncology:



Round daily on newly admitted patients with Oncology as their primary diagnosis.

Provide individualized education or counseling for patient and family prior to discharge.

Initiate post-discharge follow-up phone calls within a set parameter, screening patients for compliance Commission on Cancer (CoC) requirements.

Conduct post-discharge outpatient clinic visits as needed.



Act as a Resource for staff education.

Provide consultation as part of the Palliative Care Team for palliative care initiation and maintenance.


Performs hospital discharge follow-up counseling and education as needed.



The Navigator is to be the point of contact for patients who either self-refer or are referred from another facility for evaluation of a Oncology services and/or radiation oncology services and others services/resources as needed. The primary responsibilities of the navigator is to coordinate, navigate and educate patients in order to facilitate timely diagnosis and treatment course. Works closely with multidisciplinary team.

They will communicate closely with team/clinicians to coordinate diagnostic and therapeutic procedures. The navigator is integral to ensuring that patients move smoothly along the continuum of care once they have entered our system. The navigator will also be responsible for timely flow of information between the various clinicians involved in the care of the patient.








Qualifications:





Required License/Certification:

CA RN License

BLS

OCN Certification within 1st year of hire

BSN/MSN

Extensive oncology nurse experience, preferred as well as CoC accreditation experience

Demonstrates Clinical Competence and Leadership Skills

Demonstrated ability to present self in positive and professional manner



Qualifications:

Required: BSN/MSN, 2-5 years related work experience in Oncology Services

Required: Computer skills to include: EMR (Cerner), Microsoft Office and Web Based Applications

Preferred: Nurse Navigator experience

Preferred: Certification
