Summary

Under the clinical supervision of the Behavioral Health Director and administrative supervision of the Clinic Manager/Program Coordinator, the LCSW provides psychiatric social work assistance to psychosocially or emotionally disturbed patients and their families of self-referrals, or referred from hospitals, clinics, and other medical centers. Collaborates with psychiatric and allied teams in diagnosis and treatment plan by performing the following duties. It is the primary purpose of CHCCC to provide the highest quality, cost effective healthcare to the patient population that it serves.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

Responsible for the management, planning and coordination of Mental Health Services foe mild to moderate patients in the CHC integrated health care system. Participates in patient population management as it relates to clinical services while taking into account culturally diverse populations and limited local resources.

Participates in the team member's care coordination by providing a link between patient, psychiatric agency and the community. Interprets psychiatric treatment plan with patient and or family members. Works directly in treatment relationships with patients, individually or in groups and in consultation with service providers.

Understands the role in self management support by developing relationships with high risk clients and anticipates what needs they may encounter. Refers patients to community resources. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served (adolescent, adults or geriatric).

Develops communication skills with all members of the team both internal and external, by first being a patient advocate and using the available resources to facilitate care. Coordinate with the care team (PCP, RN, DON, or Medical Director) to ensure appropriate services are arranged for the patient in order for them to meet their self management goals in the treatment plan. Demonstrates the knowledge of domestic violence/intimate partner violence, child and dependant abuse protocols. Demonstrates adherence to and observe all safety, HIPAA and Infection control Policies and Procedures.

Understands the Quality Improvement Process which may include meetings, data collection and chart auditing. Use Evidence Based Practice guidelines to assist with disease management.

Qualifications

Current, valid LCSW license from California Board of Behavioral Examiners. Current CPR Card.
